Thank you for taking the time to look into the problem.  I was thinking the same thing that you are, that it could be a software problem
with PAF 5.

That must be a bug in the software.  I just tried running the same
procedure and the same thing happens:  I can preview the Index but
then it doesn't appear in the Word document.  I'm using PAF 5. Anyone?
